Title: Washer Hose Connector

I have a 2001 D2, the washer hose keeps coming off the T connector by the reservoir causing all the screen wash to drain out.

Is there anything I can do to fix the hose on?  I have tried some duct tape, but don't think it will work.

cut the hose back a wee bit to use an un-stretched portion?

You could try warming the end of the hose to soften it a little, and then use a cable tie to keep it tight on the connectors.  You could also try a small hose clip, either the plastic or jubilee type to keep the hose in place.
Cutting a few mm off the end of the hose would be the last choice before you replace the hose.

Mine did that, just cut off the stretched bit of pipe (about 8mm), the pipe was just long enough to do this. Refitted it and tywraped for extra piece of mind.
