Surf starting problem

Hello
Im posting this on behalf of a computer less mate of mine, he owns a 1994 2.5 diesel surf and its a real pig to start.
He has to realy churn it over before it wil eventualy start then when it does it smokes like hell for a couple of seconds (obviously the un burnt fuel)
Under my instruction he has changed the glow plug relay and also all the glow plugs but it has made no differance.
What voltage should the glow plugs be?
have you any ideas what else to look at?
once the engine has started it runs fine with no smoke and no over heating and no rattles
its got me stumped :?

Injectors- might be worth checking/cleaning these.
Fuel filter- due for a change or water trap full?
Air filter- due for a change?
Fuel pump- working

Thanks, i did try registering for the surf forum but there was a technical issue with my registration, duno why but it wouldnt let me activate my account so i am waiting for there admin to reply to my email.
i will check the injectors ect when i can work on it in daylight.

correct glow plugs? milners list 2 types for the hilux
head gasket? (check oil/water for signs of contamination)
normal running meaning normal temps, oil/water levels constant etc?
